DIALOG+ is an innovative, evidence-based clinical tool that transforms the way patients and mental health professionals communicate. Its purpose is to strengthen shared decision-making and improve people's quality of life through structured conversations focused on their priorities. Designed to be used during consultations, the app guides the patient and professional in a joint exploration of key aspects of personal life and healthcare, such as mental and physical health, relationships, work, leisure, personal safety and quality of treatment. How does it work? Personal rating The user rates their level of satisfaction in 11 essential areas. This allows you to identify strengths and opportunities for improvement in your daily life. Structured dialogue After the assessment, up to three areas are chosen to work together. The app guides a conversation in four steps: Understand the current situation Imagine an improvement Explore possible actions Define agreements to move forward Active tracking The agreed actions are recorded for follow-up in the following sessions, facilitating a continuous and person-centered process.
