Stroke is a life changing event, yet more often than not, stroke survivors leave the hospital without knowing how to transition to life after such an event. Our mission is to bridge the gap in care between hospital and home. Our AI-enabled platform identifies and monitors survivors at risk of recurrent stroke, while our clinical team guides stroke survivors through their post-stroke journey in order to maximize recovery. Please note: this app is restricted to US-based study participants at ValleyHealth in Virginia and MaineHealth in Maine.
