Cook County Health offers a more personalized health care experience through its MyCookCountyHealth app. Cook County Health patients can view their medical information, request, cancel and/or reschedule appointments, communicate with providers and much more. Participating providers can also allow you to add your HealthKit data to your electronic medical record.
