HealthPlatform Mobile is an App, enabled by healthcare facilities, which allows you to: - collect information on symptoms and responses to questionnaires - receive notifications regarding planned activities - share educational material to support patient empowerment - communicate via chat and teleconsultations with healthcare personnel The use of the App is bound by the activation by the healthcare facility which will take care of the visualization of the data sent by the patient, the processing and the intervention according to the telemonitoring service offered.
