The objective of the Pharmacotherapeutic Reconciliation Guide for patients admitted to Critical Care Units is to provide health professionals involved in the pharmacotherapy of critically ill patients with recommendations that facilitate the assessment of the benefit/risk of maintaining or suspending home medication.
