Synergy Healthcare

Synergy Healthcare App (the “App) supports employer-patients with scheduling in-person and tele-video appointments as well as secure messaging with on-site nurse-practitioners. All healthcare services by the sponsoring organization, when applicable, are delivered by licensed professionals independently of the App. The App functions solely as a technology platform facilitating communication. The App enables authorized patient-users to: securely message with authorized nurse-practitioners; schedule appointments, participate in virtual visits when enabled by the sponsoring organization, view and manage program-related information and communications. The App does not provide medical diagnosis, clinical treatment, emergency services, or medical decision-making. Some wellness features may allow optional connection to third-party consumer wellness platforms (such as Google Fit or Health Connect) to display activity and fitness participation information. These features are optional, user-authorized, and not required for core app functionality.
