This is a chart tool that combines the ICF assessment framework with a strengths-based approach. This application is suitable for users from diverse backgrounds, including those in rehabilitation and the general population. The chart tool aims to help users identify their personality strengths, connect them to relevant life examples, abilities appreciated by others, and the help and support they need. This allows for a more comprehensive understanding of themselves, while also enabling facilitators (rehabilitation service case managers, mentors, professional staff, and other family and peer groups) to understand and develop the user's strengths. The digitization of the chart tool allows users to explore and record anytime, anywhere, and easily browse, organize, and review content. It also helps facilitators to immediately grasp the user's strengths and needs, improving the efficiency of intervention, discussion, and development planning.
