The Recovery and Wellness Portal app puts your health information in the palm of your hand and helps you conveniently manage care for yourself and your family members. With the Recovery and Wellness Portal app you can: • Complete intake paperwork. • Fill out forms, sign releases, and receive records. • Respond to your care team. • Review results and other records requested. • Cancel appointments. • View and pay your medical bills. • Receive push notifications when new information is available in the Recovery and Wellness Portal. The Recovery & Wellness Centers of Midwest Ohio seek to provide quality recovery and behavior wellness services to members of Darke, Miami and Preble counties. We are certified as an outpatient addiction agency by the Ohio Department of Alcohol and Drug Addiction Services. We also offer mental health counseling for a variety of issues, community psychiatric treatment services, as well as a full scope of outpatient psychiatric services staffed by highly qualified and committed doctors and nurses.
